Family Nurse Practitioner/Physician Assistant-Rockland Co., NY

Visiting Docs - Do you enjoy helping people? Do you want to be part of a caring, compassionate team? Do you have experience working with the elderly patient population?We are looking for a motivated, hardworking, and experienced Nurse Practitioner or Physican Assistant candidate to join the growing Visiting Docs team.
The ideal candidate would have previous experience working with patients in a clinic setting and in assisted living facilities.
Responsibilities include but are not limited to:
o Providing general primary care to facility and clinic patients.
o Diagnosing, treating and managing both acute and chronic conditions.
o Prescribing and refilling medications as appropriate.
o Actively communicating with patients, families, other specialists and clinic/facility personnel (case managers, social workers, med room, etc).
o Ordering and interpreting diagnostic tests such as imaging and lab work.
o Documenting patients' histories and treatments in EMR.
o Seeing patients via telehealth, as needed.
Qualifications:
o Active unrestricted New York state NP (must have board certification) or PA license.
o Minimum three (3) years of experience preferred.
o PECOS enrolled.
o DEA certified.
o BCLS.
o Knowledge of eClinicalworks preferred.
Part time and full time schedules are available with no evenings or weekends required.
