Responsibilities:
Facilitate admissions from hospital and community setting into the facility Review referral packets from hospital with nursing department to determine if clinical needs can be met Facilitate ongoing communication with referral sources including but not limited to hospitals, assisted living facilities, community agencies and family members Provide information and tours of the facility - either in person or virtual - in order to best assist family members through the admission process Prepare both physical chart for nursing unit and complete electronic medical record Review case and obtain authorization from managed care companies Complete financial screenings to ensure insurance coverage for the facility Remain available to patients and family members during the course of their stay to ensure satisfaction Requirements:
Minimum 2 years' experience as a successful Admissions Coordinator in a nursing home facility Knowledge of Microsoft Office, typing, medical terminology knowledge and data entry required Excellent interpersonal skills; possess the ability to deal tactfully with residents, family members, visitors Associates Degree preferred We look forward to hearing from you!
